If the reset password option is not working for you, send us an email at forms@pestworld.org or give us a call and we can manually reset it for you.

Password changes when you are already logged into your account can be made under your profile. You can find your profile by selecting your name on the top right corner of the screen and selecting “my profile” from the drop down menu. Once here, you can delete any text in the password box and type in your new preferred password and retype it in the password confirm box. Remember to click save changes after you have done so. 

You can go to your profile and beneath the name and password information there is a section called signature pad. Here you can either draw in your signature or type it in in using the keyboard. What you type in will show up as a script-like font and will serve as your signature. There is also a field for your license number here that will autofill into every form you create. Make sure to save these changes. 

Yes, the NPMA 99A and 99B expiration dates have been updated as of July 2024. Prior versions of this form with the 2022 expiration date became invalid as this update was made. No contents of the form were changed.

The NPMA-33 was most recently updated in 2019. The previous version has an expiration date of 2004 and is no longer valid.  

Because the site pulls information in from google maps, you’ll need to put in a valid address for the form to save properly. You can use a nearby address such as a community office or even your company address just for this field. Within the content of the form in Section 1, you can put in the proper address. This is what will appear on the form when printed or viewed and is what matters for submission of the form to the appropriate entity.

Paper forms are available for purchase on the NPMA website at NPMApestworld.org/bugstore. These are available starting in packs of 100 and each form has carbon copies for you and your client’s records. We can also order customized forms with your company, name, and license number preprinted. 

No, the copyright laws regarding the NPMA-33 require purchase of the forms through NPMA or authorized sellers as listed on npmapestworld.org/npmaforms . 

There is a separate attachment to provide information about wood damage caused by organisms other than insects. This attachment is free and available on npmapestworld.org/npmaforms

When filling out boxes where you want to write several lines of text, you will need to hit enter on your keyboard once you have reached the end of the line as you see it on the form. The site does not recognize when to put text into the next line without having the hard return pressed to indicate so. The box in Section V can accommodate three lines of text. We also recommend using abbreviations as appropriate to shorten your comments if you are still having trouble fitting the information in.

On the Complete Forms tab of the website, the menu on the left side of the screen has an option called “Users.” Use the green “Invite” button to add a new user to the account. Once invited, they will get an email to complete the process. Accounts can have up to 20 users with access to the purchased forms or subscription from the same company.
